Reconnect

This Program provides outreach to youth from 12-18 who are at risk of becoming less connected with their families and less involved in positive activities or choices. Outreach workers encourage youth to make healthy decisions based on individual needs and help facilitate connections to appropriate services and supports as needed. Staff in this program encourage rebuilding positive family connections.
